Physical Therapist and Physical Therapist Assistant Renewal 2012

ATTENTION LICENSE, REGISTRATION OR CERTIFICATE HOLDER:  YOU ARE RESPONSIBLE FOR KNOWING THE REGULATIONS RELATED TO YOUR PROFESSION.  PLEASE READ ALL OF THE INFORMATION SUMMARIZED BELOW.

When your renewal period opens you will receive a yellow RENEWAL NOTICE POSTCARD that will direct you to this site.  There is no personal information on this postcard (other than your name, address and license number). If you have accessed this page before receipt of the postcard and renewed your license then you may disregard that notice.

Physical Therapists (PTL) and Physical Therapist Assistants (PTA) expire on October 31, 2012 and it is now time to renew your license to keep it in a current status until October 31, 2014. To renew your license, please follow the below step-by-step instructions:

Note:  In order to complete the renewal of your license, you will need to submit information in 2 separate systems (the Online Services system and the Healthcare Professions Profiling system (“HPPP”).  If you haven’t previously done so, you will be creating a Logon/User ID in each of these 2 separate systems.

1. BE PREPARED Gather needed documents or information, and set aside at least one hour of fairly relaxed and uninterrupted time to focus on your renewal. After reading all the steps detailed below, click on the link at the bottom of this page entitled "Click here to go directly to online renewal." It is a good idea to print this page and use it as a reference while you are renewing your license.

2. Healthcare Professions Profile—this requirement is new to the Physical Therapist renewal for 2012. All Physical Therapists and Physical Therapist Assistants in Active status are required to submit and keep updated a Healthcare Professions Profile ("Healthcare Profile").

• To create your Logon ID in the HPPP system, click on "Create Logon ID" at www.dora.state.co.us/pls/cproweb/CPRO.Logon. (This Logon ID will not be the same as your User ID in the Online Services system; you must create an account in each system. If you have previously created a Logon ID in the HPPP system and have forgotten it, use the "Password Finder" function.)

• Click on "Create Logon ID" on the left side of the page. You will be prompted to enter the last 4 digits of your SSN, and then enter 2 of the following: last name, birth date and/or your professional license number (not your driver's license number). Next, you will be prompted to create a Logon ID and password. Passwords are case sensitive and must be between 6 and 20 characters, contain at least one capital letter, at least one number and at least one special character such as: ! ? @ # $ % *. An example is: Pa$$word!

• After you create your Logon in the HPPP system, log in and click on "New Profile.". Answer the questions as prompted, click on "Next" to move to the next question. To add additional fields for "Yes" answers, click on "Save," click on "Done" when complete.

• Follow the prompts and then click on "Submit" to submit your Healthcare Profile. After you submit your Healthcare Profile, you can click on the message in red at the bottom of the page to go to the Online Services system.

• If you answer "Yes" to questions such as disciplinary actions, surrender of DEA registrations, felony criminal convictions, malpractice refusals, etc., you will be directed to send copies of final documentation to the HPPP office within 30 days of submitting your Healthcare Profile. Per section 24-34-110, C.R.S., this final documentation will be scanned into your profile and viewable by the public.

3. Online Services—our new Online Services system was implemented July, 1, 2012 and all users must register a new account. From this account, you can update your contact information, obtain information about your license and finish the renewal of your license.

• Click "Register" at www.colorado.gov/dora/licensing/Default.aspx to create your new online account. (This User ID will not be the same as your Logon ID in the HPPP system; you must create an account in each system.)

• You will be asked to enter your social security number and last name. The User ID field will be populated with the User ID that is assigned to you. Please write down this User ID for use in the future.

• Next, create a password, answer security questions and click "Create Account." You may select from the optional questions provided in the dropdown that will best suit your memory. These questions are used in the event that you forget your password in the future.

4. Verify Email Address—to complete the registration of your account, you may have to verify your email address.

• Once you are logged in, click on "Generate Email,", an email will be sent to the email address shown on the screen. Only click this button once. Multiple clicks will invalidate each previous email sent to you and you will have to click the link in the most recent email.

• Go to your personal email account, open the email that was just sent to you from our system, it will come from NoReply@dora.state.co.us (be sure and check your spam folder).

• Open the email you just received and click on the address link in the email (or copy and paste it into your address bar/web browser). This will take you back to our system which will validate your email.

5. Update Contact Information—after your email is validated, you can update your address of record.

• On the left side click on "Update Your Contact Information," click "Continue," enter your new address and when done click on "Finish."

6. Affidavit of Eligibility—you will need your government issued identification for this step, e.g. driver's license, passport, ID Card, etc.

• On the left side click on "Affidavit of Eligibility" click on "Start" or "Continue."

• Enter the requested information, click on "Next" to move to each next screen.

• When done, click on "Finish.". By clicking on "Finish," your Affidavit is submitted to the Division. You will not receive a message stating this is complete. (You can also click on "Close and Save" to keep your changes and come back at a later time. The Affidavit will not be submitted until you click on "Finish.")

7 . Complete Credential Renewal — On the left side, click on "Complete License Renewal."

• Click on "Start" by the appropriate license type.

• After you have reviewed the renewal fee details, click on "Add to Invoice" at the bottom of the screen.

8 . Pay Invoice—after you have completed the renewal steps for all license types and added each license type to be renewed to the invoice, you will be able to pay for your renewal. At this point, two red buttons "Pay Invoice" and "Print Invoice" will appear on the right side of the screen.

• Click on the "Pay Invoice" button. (If you want a copy of your invoice, click on "Print Invoice" before you click proceed to pay. You will also have an opportunity later to print or send an email receipt to yourself.)

9. You will pay for your renewal using our secure payment system. You must pay by Credit Card (Visa, Master Card, Discover, American Express), by Debit Card (as long as it bears the Visa or Master Card logo) or by Electronic Check (eCheck) by using your bank routing number and checking account number.

10. After entering your credit card/eCheck information and clicking "Continue" you will enter the appropriate billing information. IF YOU ENTER AN EMAIL ADDRESS, YOU WILL BE SENT AN EMAIL RECEIPT OF YOUR TRANSACTION. Make sure to complete all required fields and click "Continue." Note: The required fields are designated by a blue line.

11. You will then be able to review/verify your payment information on the following screen. If you need to make any changes/updates you can click the "edit" button in the corresponding section. If all information is correct, click the "Agree/Continue" button at the bottom of the page to complete the transaction.

12. Once complete, you will see a confirmation page. You can print this page for your records. If you entered your email address in the payment billing section, you will receive a receipt by email (be sure that the email address, support@www.colorado.gov, is added to your approved senders list to receive email confirmation; also check your spam folders). This will complete your transaction.

13. You will receive your renewed wallet license card by mail within approximately 7-10 days. To verify your new expiration date, you can select the "My User Information" tab.

License Renewal Fees:

Renewal fees have increased since the last Physical Therapy Renewal. As you may know, the renewal fee is evaluated each year and set to meet the budgetary needs of the program; however, it is difficult to predict if costs of regulation will decrease or increase in the future. Renewal fees help offset the operational expenses of the program and is an integral part of our commitment to work diligently to provide consumer protection.

  • If renewed on or before October 31, 2012 :
    • The Active Physical Therapist renewal fee is: $91
    • The Active Physical Therapist Assistant renewal fee is: $39
  • If renewed on or after November 1, 2012 , a $15 delinquency charge is applied and the totals are:
    • The Active Physical Therapist renewal fee is: $106
    • The Active Physical Therapist Assistant renewal fee is: $54
  • After December 31, 2012, your license will be lapsed and you must reinstate.

All fees are Non-Refundable. The Division of Professions and Occupations cannot and will not take payment information over the phone. We will not request credit card numbers or checking account numbers via telephone. You will only be able to pay for your renewal online, in person at our office or by mail with the necessary renewal packet. To protect your information DO NOT provide any credit card or checking account numbers over the phone to an unknown source.

Healthcare Professions Profile—The Michael Skolnik Medical Transparency Act of 2010, section 24-34-110(2)(II), C.R.S., ". . . provides transparency to the public regarding the competency of persons engaged in the practice of certain health care professions in this state to assist citizens in making informed health care decisions." The Healthcare Professions Profiling Program ("HPPP") was created to implement the Michael Skolnik Medical Transparency Act of 2010. This Act expanded the Physician Profile program (Senate Bill 10-124) to 23 professions which are now required to create and maintain Healthcare Profiles that are viewable by the public. Applicants and licensees from these healthcare professions who submit an application for an initial license, license reinstatement, reactivation, or renewal in Active or Retired status are required to disclose specific information that can be accessed by the public. BY LAW, a license in these 23 healthcare professions CANNOT be renewed in Active or Retired status unless the licensed professional complies with the profile requirements. After creating Healthcare Profiles, licensees and applicants are required to submit backup documentation for specific "Yes" answers, e.g. disciplinary actions, surrender of DEA registration, criminal convictions, malpractice refusals, etc.

Licensees are further required to update their Healthcare Profile anytime there is a change and/or a reportable event. Within 30 days of a change and/or a reportable event, please log in to the Healthcare Professions Profiling system and submit an update to your Healthcare Profile. As a Licensee, it is your responsibility to ensure that you are in compliance with this requirement. During the licensing process, the Division asked whether you have ever been disciplined in any state, arrested, charged, convicted or pled guilty to a crime. After initial licensure, all licensees have a duty to report subsequent felony convictions and disciplinary actions. Some must also report misdemeanors related to the specific practice. Failure to disclose could constitute grounds alone for revocation of your license.

Licensure is a privilege, not a right. In order to obtain and retain the privilege, you must be completely honest. The Division conducts audits of its licensing database against several criminal and national disciplinary databases, which allows the Division to track subsequent criminal conduct and disciplinary actions after initial licensure.

An arrest, disciplinary action or subsequent criminal conviction does not mean an automatic revocation of your license. The board or program will look at the facts surrounding the criminal conduct and disciplinary action to determine whether you are still fit for licensure. Your license will not necessarily be revoked if you have been disciplined, arrested, charged or convicted, but your license will most likely be revoked if you fail to disclose it.
