Non-Resident (Out-of-State) Prescription Drug Outlet (OSP) 

Definitions

• Renew:  You currently hold a Colorado license and are in a renewal period.
• Reinstate: Your Colorado license has expired and you wish to reinstate the license. 

* NOTE: You must use the same application for new registrations and changes to existing registrations
(i.e. transfer of ownership, business location or name change, or change of designated representative / consultant pharmacist / pharmacist manager). 

Forms

Any pharmacy located outside of Colorado which desires to ship a prescription into this state must first register with this Board of Pharmacy. They must provide, in addition to the application and fee, an official verification of the current pharmacy license or registration issued by the Board of Pharmacy in the state where the pharmacy is located, as well as a copy of the most recent report of inspection conducted by that state.

In addition to initial registration and renewal, an out of state pharmacy must also submit an application and fee to apply for any change of ownership, name, location and/or pharmacist manager ("pharmacist in charge").
 

License Expiration Grace Period for New Applicants. All new applicants who are issued a registration within 120 days of the upcoming renewal expiration date will be issued a registration with the subsequent expiration date.

License Renewal Information. All Non-resident Prescription Drug Outlet (OSP) registrations expire on October 31 of even-numbered years and must be renewed to continue practicing.
