Other Outlet (OO) 

Definitions

• Renew: You currently hold a Colorado license and are in a renewal period.
• Reinstate: Your Colorado license has expired and you wish to reinstate the license. 

* NOTE: You must use the same application for new registrations and changes to existing registrations (i.e. transfer of ownership, business location or name change, or change of designated representative / consultant pharmacist / pharmacist manager). 

Forms

This is a non-pharmacy outlet from which prescription drugs and/or controlled substances are dispensed to patients of the outlet. The following types of outlets may qualify for registration by the Board of Pharmacy: jails; county health departments; community and rural health clinics; college and university student/faculty health services; hospitals which do not operate prescription drug outlets ("pharmacy").

A licensed pharmacist must make an initial interpretation of all prescriptions dispensed from an Other Outlet, or a Consultant Pharmacist must provide written, board-approved protocols for such dispensing by non-pharmacists. The Consultant Pharmacist must also perform inspections of the Other Outlet to assure compliance with the protocols, as well as such other duties as are explained in the Board's regulations.In addition to initial registration and renewal, an Other Outlet must submit an application and fee to apply for any change of ownership. An application must also be submitted for any change in name, location and/or Consultant Pharmacist, although no fee is associated with these changes.

License Expiration Grace Period for New Applicants. All new applicants who are issued a license within 120 days of the upcoming renewal expiration date will be issued a license with the subsequent expiration date.

License Renewal Information. All Other Outlet registrations expire on October 31 of even-numbered years and must be renewed to continue practicing.
