Whether the pharmacy is to be located in a retail or hospital setting, or if it is a specialty pharmacy, such as one devoted to dispensing sterile parenteral products or a nuclear pharmacy dispensing radiopharmaceuticals, the application and registration is the same. Or, put another way, one single pharmacy registration permits a variety of types of pharmacy practice to be conducted within the outlet.
In addition to initial registration and renewal, a pharmacy must also submit an application and fee to apply for any change of ownership, name, location and/or pharmacist manager ("pharmacist in charge"), as well as to remodel its compounding/dispensing area or to open a satellite facility.
License Expiration Grace Period for New Applicants. All new applicants who are issued a license within 120 days of the upcoming renewal expiration date will be issued a license with the subsequent expiration date.
License Renewal Information. All Prescription Drug Outlet registrations expire on October 31 of even-numbered years and must be renewed to continue practicing.
