Limited License (LTD)

Definitions

• Renew: You currently hold a Colorado license and are in a renewal period.
• Reinstate: Your Colorado license has expired and you wish to reinstate the license.

* NOTE: You must use the same application for new registrations and changes to existing registrations
(i.e. transfer of ownership, business location or name change, or change of designated representative / consultant pharmacist / pharmacist manager).

Forms

The "Limited License" is issued to a nonprofit humane society and a government-operated animal control agency. It permits the society or agency to obtain a registration with the Drug Enforcement Administration to obtain sodium pentobarbital, alone or in combination with other non-controlled substances, to euthanize injured, sick, homeless or unwanted pets and animals.

License Expiration Grace Period for New Applicants. All new applicants who are issued a license within 120 days of the upcoming renewal expiration date will be issued a license with the subsequent expiration date.

License Renewal Information. All Limited License registrations expire on October 31 of even-numbered years and must be renewed to continue practicing.
