Water Annual Report Forms

Energy /Water General Reporting Forms

 

Utility Designee /Designated Agent

  

The Utility Designee form must be completed by all utilities upon application approval and be updated as utility information changes.

The Commission has opened a repository proceeding, Proceeding No. 15M-Designee, to facilitate parties E-Filing their Utlity Designee form. This proceeding number is to be used this year with an initial or update Utility Designee filing. The appropriate Utility Designee form must be completed by all utilities upon application approval and be updated as utility information changes.

The E-Filing “Filer Administrator” or one Designated Agent (if a company does not have an E-Filing “Filer Administrator”) will receive filings that require official service from the PUC.

 

 

Energy /Water Annual Reporting

 


 Annual Reports are due April 30th of each year for the previous calendar year. Appropriate forms are provided to the annual report contact for each utility.

  • The Department of Revenue form DR 525  -  "Annual Report of Public Utility Intrastate Gross Operations Revenue" - must be filed in addition to the Annual Report required by this Commission for maintaining your certification or registration and is due to the Commission by April 30th of each year for the prior calendar year's gross Colorado Regulated Operating Revenues.

This DR 525 report MUST also be filed with the Colorado Department of Revenue on or before May 1st of the reporting year.

Cooperative electric associations subject to Article 9.5 of Title 40, C.R.S., and all energy utilities must also file a copy of the report of gross regulated intrastate operating revenues required by the Colorado Department of Revenue, Form DR 0525.

Federal Energy Regulatory Commission Annual Report Forms for Electric Utilities

 

Annual Report of Major Electric Utility - FERC Form No. 1: blank form pdf file

  • The Form No. 1 is a comprehensive financial and operating report submitted for Electric Rate regulation and financial audits. Major is defined as having (1) one million Megawatt hours or more; (2) 100 megawatt hours of annual sales for resale; (3) 500 megawatt hours of annual power exchange delivered; or (4) 500 megawatt hours of annual wheeling for others (deliveries plus losses).

Colorado PUC Annual Reporting Forms for Electric Utilities

In accordance with Rule 3006 of the Commission's Rules Regulating Electric Utilities, 4 Code of
Colorado Regulations (CCR) 723-3, each utility over which the Commission has jurisdiction is required to file an annual report on or before April 30th for the preceding calendar year. The rules state that the annual report shall be filed on forms prescribed by the Commission and require that regulated utilities file any additional reports specified by the Commission.

Please see attached Appendix A for a list of energy utilities and the required reports for each utility. The attachment specifies the forms to be filed by energy and steam utilities for the reporting year.

Federal Energy Regulatory Commission Annual Report Forms
for Natural Gas Utilities

Major Natural Gas Company FERC Annual Report Form 2 blank pdf file  

  • The Form No. 2 is a compilation of financial and operational information from major interstate natural gas pipelines subject to the jurisdiction of the FERC. The form contains data for a calendar year. Among other things, the form contains a Comparative Balance Sheet, Statement of Income, Statement of Retained Earnings, Statement of Cash Flows, and Notes to Financial Statements.

Form No. 2A is an abbreviated Form No. 2 filed by non-major interstate natural gas pipelines subject to the jurisdiction of the FERC.

Oil Product Pipeline Company Annual Report Form  
 

  • reporting forms are mailed or emailed to affected companies

On or before April 30 each year, each Oil Product Pipeline Company operating in intrastate commerce within the State of Colorado during the preceding calendar year shall file with the Commission an annual report on the form prescribed in Commission letter they receive by email or US Mail.

 

Each Pipeline Company shall also file a copy of the DR 0525 - "Annual Report of Public Utility Intrastate Gross Operations Revenue" pdf file  no later than April 30 each year.  Companies may not request an extension of time to file Form DR 0525.

Please file with the Commission as follows: 

Companies are strongly encouraged to have one of their designated filers submit an executable electronic copy (i.e., MS Excel workbook) in the appropriate proceeding number (Proceeding No. 14M-0052W) via the Commission’s E-Filings system at www.dora.state.co.us/pls/efi/EFI.homepage 

Once in the E-Filings system, select the document type “Annual Report” and in the title field insert the utility’s name, along with “2014 Annual Report". E-filing is a fast, efficient, and less costly way to submit filings to the Public Utilities Commission. 

In the alternative, mail the signed original, one copy, and an executable electronic file on CD. Mail the hard copies and CD to the following address:
